A 62-year-old man born in rural Kerala (India), resident in the UK for 20 years, is admitted with abrupt-onset Gram-negative septicaemia, ARDS, and meningitis, 10 days after starting high-dose corticosteroids for a suspected exacerbation of interstitial lung disease. Blood cultures grow Escherichia coli; CSF shows a neutrophilic pleocytosis with Gram-negative rods. He is intubated and ventilated. Sputum and stool both show numerous filariform larvae.
