Core knowledge (model answer backbone)
Mitral valve prolapse (MVP) is the systolic billowing of one or both mitral leaflets above the mitral annulus into the left atrium, defined echocardiographically as leaflet displacement of more than 2 mm beyond the mitral annular plane in the parasternal long-axis view during systole, with a maximal leaflet thickness of at least 5 mm defining the classic form. The underlying pathology is myxomatous degeneration of the leaflet (proteoglycan-rich matrix, fragmented collagen) with chordal elongation and often annular dilatation or mitral annular disjunction (MAD). MVP is the commonest primary valve abnormality in developed countries, with a population prevalence of 2 to 3 percent — 2.4 percent in the Framingham offspring cohort, where the age and sex distributions of those with prolapse matched those without, so the often-quoted 2:1 female predominance belongs to referral series rather than to the community. Two morphological phenotypes are recognised: fibroelastic deficiency (thin leaflets, focal prolapse, often the P2 scallop, sudden chordal rupture, acute MR, typically the older patient) and diffuse myxomatous Barlow's disease (multisegmental billowing, bileaflet, redundant tissue, marked annular dilatation, chronic severe MR, typically the younger patient). Most patients are asymptomatic; the classic bedside signature is the mid-systolic click and late systolic murmur that responds to dynamic manoeuvres — the click moves earlier with Valsalva and standing (reduced preload, so the leaflet prolapses sooner) and later with squatting or handgrip (raised preload, so it prolapses later), which is the opposite of the HOCM murmur's response. Management is risk-stratified: reassurance and surveillance for the asymptomatic, beta-blockade for symptomatic palpitations or atypical chest pain, no endocarditis prophylaxis, and mitral valve repair (preferred over replacement) once severe primary MR reaches a Class 1 trigger.[2][1][7]
Red flags
- Sudden syncope, cardiac arrest or polymorphic VT/VF in a young patient with MVP and MAD - arrhythmic MVP; secondary prevention, ICD, treat MAD/MR
- New acute severe MR after chordal rupture - pulmonary oedema and cardiogenic shock; urgent surgery (repair), vasodilator bridge
- New neurological deficit in MVP with AF - cardioembolic stroke; anticoagulate, rate/rhythm control, treat MR
- Progressive breathlessness, rising pulmonary pressures or LVEF 60 percent or less - severe MR reaching the surgery threshold; refer to valve clinic
- Endocarditis in MVP with new MR, embolic phenomena or abscess - intravenous antibiotics, consider early surgery

Key doses / thresholds
- Surgical triggers in severe primary MR (2020 ACC/AHA): Class 1 if symptomatic, irrespective of LV systolic function; Class 1 if asymptomatic with LVEF 60 percent or less or LVESD 40 mm or more. Note the inclusive boundaries.[1]
- Echo criteria: leaflet displacement more than 2 mm beyond the annular plane; leaflet thickness 5 mm or more for the classic form; mitral annular disjunction more than 8.5 mm predicts non-sustained VT.[2][3]
- Beta-blockade for symptomatic palpitations or atypical chest pain (off-label in MVP; the original evidence is propranolol, which improved symptoms in 6 of 16 patients, almost entirely through less palpitation): metoprolol tartrate 25 to 100 mg twice daily titrated to response, or bisoprolol fumarate 2.5 to 10 mg once daily. The bisoprolol label starts at 5 mg once daily, with 2.5 mg an appropriate start in some patients, increasing to 10 mg then 20 mg if needed; the metoprolol tartrate label's studied oral range is 100 to 400 mg daily in divided doses.[4][8]
- Verapamil when a beta-blocker is not tolerated: immediate-release 80 to 120 mg three times daily per the label's angina range, and never exceeding 480 mg daily.[8]
- Oral amiodarone for ventricular arrhythmia, per the FDA label: load 800 to 1600 mg daily until response (usually 1 to 3 weeks), reduce to 600 to 800 mg daily for one month, then maintenance usually 400 mg daily. Divide doses with meals at 1000 mg daily or more.[8]
- Out-of-hospital shock-refractory VF or pulseless VT (ALPS protocol): up to 450 mg amiodarone IV or up to 180 mg lidocaine IV; neither improved survival to discharge over placebo overall.[5]
- Endocarditis prophylaxis: none for MVP at any degree of MR - MVP is not one of the highest-risk conditions in the AHA/ACC list, and NICE CG64 does not recommend routine prophylaxis for dental procedures. Amoxicillin 2 g orally 30 to 60 minutes before a dental procedure applies only to the listed highest-risk patients, which includes an MVP patient after mitral repair with an annuloplasty ring; clindamycin is no longer an accepted alternative for penicillin allergy.[1][6]
- Bedside manoeuvres: the mid-systolic click moves earlier and the late systolic murmur lengthens on standing or Valsalva as LV volume falls, and later on squatting or handgrip - the opposite of the HOCM murmur.[7]

e) Immediate resuscitation and definitive management with doses where standard. (3 marks)
- ABC / time-critical steps first.
- First-line drug(s) with agent + dose + route (or procedure steps).
- Escalation triggers (theatre, ICU, thrombolysis window, antidote, etc.).
- Disposition and safety-netting.
Marking tips
Full marks require specificity (numbers, names, doses) not generic "give antibiotics/fluids." Regional practice (ICMR / NICE / AHA) may be cited as alternative where relevant.
